The 3rd Annual Virginia Mus
eum of Natural History Foundation Mixed Captain’s Choice Golf Tournament will be held on October 3, 2008, at Westlake Country Club, Hardy, Virginia. The PGA Pro will be Billy McBride
III.
Awards include gifts for the 1st, 2nd, and 3rd place group or set based on net scoring. Participants with the closest tee shot to the pin on the Par 3s and longest drives receive prizes.

The entry fee is $65.00 per person or $260.00 per team, and includes greens fee, cart fee, lunch before play and refreshments during play. $5.00 per person for a M
ulligan and a free throw. (Limit 2 per person.)
Deadline for entry is Monday, September 29, 2008. Entries will be taken if space is available until the morning of the tournament.

Title Sponsorship
$5,000
- This sponsorship allows a business to partner with the VMNH Foundation and be listed in the title of the event in all marketing materials, advertising and signage. Includes four four-player teams, free range balls, and lunch.
Premier Sponsorship
$2,000
- A premier sponsorship includes three four-player teams, a sign on one hole promoting your business, main signage, free range balls, and lunch.
Exclusive Hole Sponsorship
$1,000
- Sponsorship includes two four-player teams, exclusive sponsorship for one hole, free range balls, and lunch.
Team Sponsorship
$500
- Team sponsorship includes one four-player team, a sign promoting your business on sponsor’s hole of choice (first come, first served), free range balls, and lunch.
Single-Hole Sponsorship
$100
- Sponsorship includes signage on hole promoting your business and lunch.
